School vaccination program
Is your child in Year 7 or Year 10? Provide your consent for their routine school vaccinations online.
In Year 7, students are offered vaccines for diphtheria-tetanus-pertussis (dTpa) and human papillomavirus (HPV). In Year 10, students are offered the meningococcal ACWY vaccine.
Vaccination clinics will be conducted at Colo High School on the following dates:
Visit 1: Wednesday 3 May 2023:
- Year 7 HPV - all students.
- Catch up will be offered at this clinic to any students who provided a consent in 2021 or 2022 but missed any vaccine(s).
Visit 2: Friday 2 June 2023:
- Year 10, Meningococcal ACWY
- Catch up will be offered at this clinic to any Year 7 students who missed any vaccine(s) at visit 1.
- Catch up will also be offered to any students who provided a consent in 2021 or 2022 but missed any vaccine(s).
Visit 3: Thursday 24 August 2023:
- Year 7, dTpa- all students
To provide online consent you will need:
• To scan the QR code below
• Your Service NSW log-in details
• Medicare card details for you and your child
If you or your child do not have a Medicare card, consent can still be provided by requesting a paper-based consent form directly from our school.
